Complete and submit the prescribed application form along with the following documents
- List of names and reference of certificates of competency for each Master and Deckhand
- Copy of valid Certificate of Insurance for each vessel
- Copy of Seaworthiness Certificate of vessels to be used
- List of fares for requested route
- Recommendation of applicant’s fitness to hold a permit
- Payment of application fee
